For healthcare and locum agencies
Formatix AI fills the forms healthcare and locum agencies send with every booking, such as locum application forms, assignment checklists and nurse or AHP candidate profiles, from the CV, registration documents and your notes. It fills what those sources contain and leaves the rest blank, with a list of what is still needed.
No credit card. Your first ten CVs are free, and the trial credits are valid for 14 days.
The same details, typed again for every booking
Agencies supplying the NHS and other healthcare providers send a form with each worker: an application form, an assignment or placement checklist, or a candidate profile in the client's layout. They ask for the same things each time, such as registration, checks, training, employment history and references.
All of it is already in the CV and the compliance file. The work is re-typing it into the next client's form, booking after booking, and making sure nothing is missed.
How it works
- 1
Load the form once
Upload the form your client or framework issues, in Word or Excel, or start from a Formatix template below. Formatix builds a template that keeps its layout, and you check a filled sample.
- 2
Send in the documents
The CV, plus registration and compliance documents and your notes. Upload them, email them to [email protected], let new CVs come in from JobAdder, Salesforce or HubSpot, or work from inside ChatGPT or Claude.
- 3
Check, complete and send
The filled form opens beside the sources. Complete the blank fields, such as signatures and declarations, and download it in Word or PDF.
Formatix copies what your documents say. It does not check registrations, DBS or right to work, and it never fills a declaration on anyone's behalf.

Templates to start from
Each one is rebuilt in your brand. Or upload your own format, or the one your client sends you, and Formatix builds the template from it.
WordLocum doctor application formThe detailed application form a locum agency completes for a doctor before submitting them to an NHS trust: GMC number and qualifications, full employment history with gaps explained, each post in detail (months per department, hospital information, departments worked, daily duties, procedures per week), career totals per procedure, the self-declaration procedures checklist, the consolidated summary of experience and a list of what is still needed. Ticks and numbers only as stated. Not an official form.
WordNHS clinical agency assignment checklistThe temporary worker assignment checklist a supplier completes for every booking with an NHS trust under the RM6281 Clinical and Healthcare Staffing framework: the Buyer's IR35 status determination, contractual and booking details, the worker, registration and qualifications, mandatory training, charge rate and supplier fee, PAYE route, conflicts, validated documents, enhanced DBS, occupational health, the supplier declaration and what is still needed. Ticks only as the documents confirm. Structure based on the RM6281 assignment checklist (Open Government Licence v3.0). Not an official form.
WordNHS non-clinical agency assignment checklistThe temporary worker assignment checklist a supplier completes for every booking of an administrative, corporate, IT, legal, scientific or estates worker under the RM6277 Non-Clinical Staffing framework: the Buyer's IR35 status determination, contractual and booking details, the worker, charge rate and supplier fee, PAYE route, conflicts, validated documents, DBS, occupational health, the supplier declaration and what is still needed. Ticks only as the documents confirm. Structure based on the RM6277 assignment checklist (Open Government Licence v3.0). Not an official form.
WordAgency worker placement checklistThe checklist an agency completes for each placement it puts to an NHS trust, in three variants by staff group (medical locum, AHP / healthcare science, nursing) that switch the registration and training rows: booking, grade or band, dates, hours and on-call, pay and agency fee only as stated, CV, photo, ID and right to work, registration and last check, DBS, occupational health, life support and appraisal or mandatory training, English, two references, a still-needed list and a blank supplier sign-off. Ticks only as stated. Not an official form.
WordLocum CV compliance reviewA one-to-two-page review of a locum doctor's CV against what trusts require before a doctor is deemed suitable: full name, availability, clinical and specialty experience, gaps explained, GMC registration type, number, currency and restrictions, DBS, two recent references and hepatitis B evidence, each marked Present / Missing / Unclear from the CV with the evidence quoted or what is missing. Ready to submit is ticked only when every requirement is Present; a still-needed list follows. Not an official form.
WordEnd-of-assignment reportThe end of assignment assessment report a Buyer completes on a temporary worker, for suppliers on the RM6281 clinical and RM6277 non-clinical staffing frameworks: report details, overall performance, whether the Buyer would engage the worker again, comments and training needs, the reporting officer, the worker's statement and the detailed assessment of fourteen criteria, plus what is still needed from the Buyer. Ratings only as the Buyer gave them. Structure based on the RM6281 and RM6277 end of assignment reports (Open Government Licence v3.0). Not an official form.
WordNurse and allied health candidate profileA placement-ready profile an agency sends to an NHS trust for a nurse, midwife or allied health professional, built around what the trust checks at placement: the professional register and PIN in a boxed register summary with the last check and revalidation date, then a checks table (enhanced DBS and the update service, occupational health, right to work, English) with result, date and reference, then clinical areas and skills, life support and mandatory training with completion and expiry dates, the employment history by trust, ward and band with every gap shown, education, references and what is still needed. Only as stated; not an official form.
WordCare worker profileA one-page profile of a care worker or healthcare assistant for care homes, home care and social care bookings: the questions a manager asks answered first in plain words (availability, shifts, DBS with workforce and update service, driving, Care Certificate and NVQ / QCF / diploma level), care settings, experience, languages, mandatory training with dates and up to three recent roles. Exactly one page per worker; nothing beyond the CV and notes.
WordHealthcare CVAn agency CV for nurses, allied health professionals and doctors: a registration card (register, PIN or number, revalidation, DBS, only as stated), clinical areas, mandatory training with dates, education and languages beside the profile, clinical skills, employment by trust and unit, and rotations or placements.
WordClient submittalA cover document that puts a candidate forward to a client: why they fit the brief, key facts, terms and next steps.
Frequently asked questions
- Are these the official NHS or framework forms?
- No. They are Formatix templates for suppliers, built to follow the structure of the forms agencies are asked to complete. They are not official forms and carry no NHS or government logo. If your client issues its own form, upload it and Formatix builds the template from that.
- Does Formatix check registrations, DBS or right to work?
- No. It copies what your documents say into the form and leaves anything they don't contain blank. Checks and declarations stay with your compliance team.
- Where are the documents kept?
- Formatix is UK-hosted on AWS London. Team owners can set records to delete automatically after 30, 60, 90 or 365 days, records can be deleted at any time, and documents are never used to train AI models.
- Does it work with our ATS?
- JobAdder, Salesforce, HubSpot and Ezekia have native connectors on every plan, including the free trial. Any other ATS or CRM works by email: send the CV to [email protected] and the filled document comes back by reply.
