    NHS non-clinical agency assignment checklist

The non-clinical temporary worker assignment checklist is the form a supplier completes for every booking of an administrative, corporate, IT, legal, scientific or estates worker under the RM6277 framework. Formatix fills it from the worker's CV, the Buyer's order and your compliance notes, copies the Buyer's IR35 determination and every rate exactly as stated, ticks a check only when your records confirm it, and lists what you still need.

No credit card. Your first ten CVs are free, and the trial credits are valid for 14 days.

    What it is

The RM6277 Non-Clinical Temporary and Fixed Term Staff framework is used by NHS, local government and central government buyers. Suppliers complete an assignment checklist for each booking: the Buyer's IR35 status determination, the contract, the requirement, the worker, pay and the PAYE route, conflicts, the validated documents, the DBS check, occupational health and the supplier's declaration.

The Formatix template follows that structure in your own brand, with no NHS, framework or government logo, and adds a Still needed box at the end. Unlike the clinical checklist, it has no registration, qualifications or mandatory training sections.

Ticks, rates and dates come only from the worker's documents, the order and your records. Anything missing is left blank and added to the Still needed list.

Signature on the supplier declarationAlways left blank for your authorised signatory.

A check your records do not confirmIt stays unticked and goes on the Still needed list; it is never marked No by default.
A rate or fee that is not statedA fee is never worked out from a charge rate, and a night or weekend rate never from a day rate.
Date of birth, title or gender not statedNever inferred from a name; right to work is never inferred from nationality.

    The Still needed list

The checklist ends with a "Still needed before submission" box for the agency. It lists, most important first, every item the checklist asks for that the documents do not give: for example the Buyer's IR35 determination and reasons, whether a DBS check is required for the role, the DBS certificate number, or the night and weekend rates. Obtain them, then remove the box before sending.
