Nurse and allied health candidate profile template
A nurse and allied health candidate profile is the placement-ready profile an agency sends to an NHS trust for a nurse, midwife, allied health professional or healthcare scientist. This Word template is built around what the trust checks at placement, in the agency's brand: the name, profession, band, location and availability; the professional register in one ruled box (register and PIN, what the candidate is registered as, the last register check and revalidation); placement checks as a table of check, result as stated, date and reference (enhanced DBS and the update service, occupational health, right to work and English); clinical areas and skills; life support and statutory and mandatory training with completion and expiry dates; the employment history by trust, ward and band, with every gap shown; education; references; and a Still needed list. Formatix fills it from the candidate's CV and your compliance notes, and every check appears only as those documents state it. It is a Formatix template, not an official form.
A trust's booking team reads the checks before the CV. This profile puts them first, the register in its own box and the other checks in a table, and then sets out training as dated tables and the employment history with every gap shown. Nothing is calculated: an expiry date or a revalidation date appears only when it is stated, and anything missing shows "Not stated" and goes on the Still needed list.

Questions about this template
- Will it calculate training expiry or revalidation dates?
- No. Expiry dates and the revalidation date appear only as stated; they are never worked out from a completion date or a usual renewal period. The last register check appears only when the documents say when the register was checked.
- Does Formatix check the register or the DBS?
- No. It copies what the candidate or your compliance file states, and the profile notes that the trust should verify the register entry and the DBS certificate before the placement starts.
- How are gaps in employment shown?
- Every post is listed, most recent first. A gap of more than a month is shown with its dates and the reason as stated, or "reason not stated", in which case the reason goes on the Still needed list.
- How is it different from the healthcare CV?
- The healthcare CV is a two-column CV with a registration card in a sidebar. This is a single-column placement profile with the register and placement checks first, training as dated tables and the employment history as a table by trust, ward and band. It has no photo.
