A site will not let an operative start without the right cards in date, and the client's supply chain team wants the checks behind each booking. The details sit in card photos, certificates, Smart Check results and Sentinel prints.

Retyping them into a profile, a checklist and a spreadsheet for every operative is where a red card becomes a blue one or an expiry date slips.

How it works

   Start from a template below, or upload your own profile or the pre-start checklist your client issues. Formatix builds a template that keeps its layout, and you check a filled sample.

   Send in the documents

   The CV with card scans, certificates, Smart Check results and Sentinel prints, by upload, by email to [email protected], from JobAdder, Salesforce or HubSpot, or from inside ChatGPT or Claude.

   Check and send

   Each document opens beside the originals for checking. Chase what is on the Still needed list, then download it in Word, Excel or PDF.

Formatix copies what the documents say. It does not run CSCS Smart Checks, Sentinel checks or right to work checks, and it never marks a card as valid.

Frequently asked questions

Are these official contractor or scheme forms?

No. They are Formatix templates and say so in their footers. Card and competence names follow the CSCS, CPCS, NPORS, CITB and Sentinel information pages. If your client issues its own form, upload it and Formatix builds the template from that.

Does Formatix check cards on CSCS Smart Check or Sentinel?

No. It copies what the documents and your records say, and never marks a card or competence as valid. A card is marked Expired only when the documents say so or a stated expiry date has passed.

Where are the documents kept?

Formatix is UK-hosted on AWS London. Team owners can set records to delete automatically after 30, 60, 90 or 365 days, records can be deleted at any time, and documents are never used to train AI models.

Does it work with our ATS?

JobAdder, Salesforce, HubSpot and Ezekia have native connectors on every plan, including the free trial. Any other ATS or CRM works by email: send the CV to [email protected] and the filled document comes back by reply.
