Healthcare and locum agencies
Nurse and allied health candidate profile
A nurse and allied health candidate profile puts a nurse, midwife, allied health professional or healthcare scientist forward for a trust placement with the placement checks first. Formatix fills it from the candidate's CV and your compliance notes, copies registration, DBS and training dates exactly as stated, shows "Not stated" for any check not given, and lists what is still needed.
No credit card. Your first ten CVs are free, and the trial credits are valid for 14 days.
What it is
A trust's booking team checks the register entry, DBS, occupational health, right to work, English, training and references before a placement. This profile is built around those checks.
The Formatix template has the name, profession, band, location and availability; the professional register in one ruled box; placement checks as a table (check, result as stated, date, reference); clinical areas and skills; life support and statutory and mandatory training as dated tables; the employment history by trust, ward and band with every gap shown; education; references; and a Still needed box. It has no photo and no trust logo.
Every check and date comes only from the documents. The profile notes that the trust should verify the register entry and the DBS certificate before the placement starts.

What stays blank
| Field | Why it stays blank |
|---|---|
| A placement check that is not stated | It shows "Not stated" and the check goes on the Still needed list. |
| An expiry or revalidation date that is not stated | Never worked out from a completion date, a registration date or a usual renewal period. |
| Last register check, when no date is given | Never set to today's date. |
| Right to work or English competence, when not stated | Never inferred from nationality, a name, a university or a country. |
The Still needed list
The profile ends with a "Still needed before placement" box for the agency, listing, most important first, every placement check the documents do not give: for example the date the register entry was last checked, the DBS certificate number and update service status, occupational health clearance, the reason for a gap or a second reference. Obtain them, then remove the box before sending the profile.
