Healthcare and locum agencies
NHS clinical agency assignment checklist
The temporary worker assignment checklist is the form a supplier completes for every booking with an NHS trust under the RM6281 Clinical and Healthcare Staffing framework. Formatix fills it from the worker's CV, the trust's order and your compliance notes, copies the Buyer's IR35 determination and every rate exactly as stated, ticks a check only when your records confirm it, and lists what you still need before the booking.
No credit card. Your first ten CVs are free, and the trial credits are valid for 14 days.
What it is
Suppliers on the RM6281 framework complete an assignment checklist for each booking. It starts with the Buyer's IR35 status determination statement and covers the contract, the requirement, the worker, their registration, qualifications and mandatory training, pay and the PAYE route, conflicts, the validated documents, the enhanced DBS check and occupational health, and ends with the supplier's declaration.
The Formatix template follows that structure in your own brand, with no NHS, framework or government logo, and adds a Still needed box for the agency at the end.
Ticks, rates, dates and numbers on this checklist come only from the worker's documents, the order and your records. Anything missing is left blank and added to the Still needed list.
What Formatix fills in
| Field | Filled from |
|---|---|
| The Buyer's IR35 status determination, reasons and disputes address | The Buyer's order or your notes, only as the Buyer stated them |
| Buyer and supplier names and references, framework lot, call-off contract | The booking or your notes |
| Job title, band or grade, working pattern, dates, reason for the booking | The booking, as stated |
| Worker's name, title, date of birth, NI number, previous work for the Buyer | The CV or your notes, only if stated |
| Registration body, number, type and renewal; qualifications | The CV or your compliance record, as stated |
| Mandatory training: course, date completed, practical given | Training records or the CV, only as stated |
| Hourly charge rate and supplier fee for days, nights and Saturdays, Sundays and bank holidays | Your notes, copied exactly |
| PAYE route and the party operating PAYE | Your notes, only as stated |
| Conflicts of interest and concurrent placements | Your notes, Yes or No only as stated |
| Checks: identity, right to work, employment history, photograph, references, fitness, BPSS, visa, English, training | Ticked only when your notes confirm the check was completed and the validated document is held |
| DBS certificate number, issue date, update service and last update check; occupational health provider and date | Your compliance record, as stated |
| The Buyer's additional requirements | The Buyer's order, as stated |
| Supplier declaration: name, position, date | Your notes, only when they say who signs and when |
What stays blank
| Field | Why it stays blank |
|---|---|
| Signature on the supplier declaration | Always left blank for your authorised signatory. |
| Inside or outside IR35, when the Buyer's determination is not given | Formatix never decides IR35 status or works it out from the pay route. The inside / outside boxes stay unticked and the matching statement is printed only once the determination is stated. |
| A check your records do not confirm | It stays unticked and goes on the Still needed list; it is never marked No by default. |
| A rate or fee that is not stated | A fee is never worked out from a charge rate, and a night or weekend rate never from a day rate. |
| Date of birth, title or gender not stated | Never inferred from a name; right to work is never inferred from nationality. |
The Still needed list
The checklist ends with a "Still needed before submission" box for the agency. It lists, most important first, every item the checklist asks for that the documents do not give: for example the Buyer's IR35 determination and reasons, the DBS certificate number and issue date, occupational health clearance, or the night and weekend rates. Obtain them, then remove the box before sending.
