End-of-assignment report
An end of assignment assessment report records the Buyer's view of a temporary worker when an assignment ends, for the supplier's file and the worker. Formatix fills it from the Buyer's completed form, an email from the manager or your notes of a call, ticks only the ratings the Buyer gave, and lists what is still needed from the Buyer or the worker.
No credit card. Your first ten CVs are free, and the trial credits are valid for 14 days.
What it is
Under the RM6281 clinical and RM6277 non-clinical staffing frameworks, the Buyer completes an end of assignment report on the temporary worker: overall performance, whether they would engage the worker again, comments, training needs and a detailed assessment of fourteen criteria, with statements from the Buyer's reporting officer and the worker.
The Formatix template follows that structure in one design for both frameworks, in your brand, with no NHS, framework or government logo, and adds a Still needed box for the supplier.
Every rating and comment is the Buyer's own. A criterion the Buyer did not rate stays blank.
What Formatix fills in
| Field | Filled from |
|---|---|
| Framework, worker, report date, supplier, buyer and reference, position, assignment dates | The Buyer's report, the booking or your notes; the framework only when named |
| Overall performance: Excellent, Good, Satisfactory or Unsatisfactory | Only as the Buyer rated it |
| Would engage again | Yes or No only as the Buyer stated |
| Comments and training needs | The Buyer's words, tidied for spelling only |
| Reporting officer: name, position, date | The Buyer's report or your notes |
| Worker's statement: agree or disagree, name, position, date | Only when the worker has completed it |
| Fourteen criteria, from record taking to manners | Each rated only when the Buyer rated it |
What stays blank
| Field | Why it stays blank |
|---|---|
| Signatures of the reporting officer and the worker | Always left blank. |
| A criterion the Buyer did not rate | Never inferred from a comment's tone or the overall rating, and never averaged. |
| The framework, when none is named | Never guessed from the role. |
The Still needed list
The report includes a "Still needed from the Buyer" box for the supplier: what the report still needs from the Buyer or the worker, most important first, such as the reporting officer's name and position, the report date, ratings for criteria not rated or the worker's statement. When performance is Unsatisfactory, every unrated criterion is listed, because the full assessment is needed. Obtain them, then remove the box.
