Healthcare and locum agencies
Agency worker placement checklist
An agency worker placement checklist confirms the booking, pay, documents, registration, DBS, occupational health, training and references for each worker an agency places with an NHS trust. Formatix fills it from the worker's CV, the booking and your compliance records, switches the registration and training rows by staff group, ticks an answer only when the documents give it, and lists what is still missing.

What it is
Trusts ask agencies to confirm a set of checks for every placement. The registration and training checks differ by staff group: a medical locum needs GMC or GDC registration, life support and the last appraisal; allied health and healthcare science staff need HCPC or another regulator, life support and mandatory training; nurses need NMC registration, life support, mandatory training and revalidation.
The Formatix template is a generic checklist with those three variants, chosen by staff group. It covers the placement, pay and charges, documents and checks, professional registration, DBS and occupational health, training, references, a Still needed box and a supplier declaration with a blank sign-off.
Every Yes or No comes only from the documents. If a trust gives you its own checklist, upload it and Formatix builds the template from that instead.
What Formatix fills in
| Field | Filled from |
|---|---|
| Worker, staff group, agency, trust and reference | The CV and the booking; the staff group only when the profession is clear |
| Reason, grade or band, job title, specialty, dates, working pattern, hours, on-call | The booking, as stated |
| Hourly pay rate, agency fee, total hourly charge, other costs | The booking or your notes, copied exactly with the currency sign |
| Full CV, photograph, ID, right to work, previous work at the trust, available for the full period, English | Yes or No only when your records say so, with the details as stated |
| Registration number, type, last checked, conditions, alert notification | Your compliance record, copied character for character |
| DBS type, number, issue date, obtained by, update service, barred list check date; occupational health provider and date | Your compliance record, as stated |
| Life support, last appraisal and responsible officer, mandatory training, revalidation | Records or the CV, only the rows for the worker's staff group |
| Two references: referee, role and organisation, period covered | Your records, as stated |
What stays blank
| Field | Why it stays blank |
|---|---|
| Supplier sign-off | Left blank for completion by hand. |
| A Yes or No the documents do not give | The box stays unticked. It is never answered No because something is not mentioned, or Yes because it is usual. |
| A pay figure that is not stated | None is worked out from another, and VAT is never added. |
| Staff group, when the profession is not clear | Left unticked; the checklist then shows every registration and training row. |
The Still needed list
When anything is missing, the checklist prints a "Still needed before submission" box for the agency, listing, most important first, every check the documents do not give: for example a second reference, the DBS certificate number and issue date, the occupational health certificate, the registration check date or the agency fee. Obtain them, then remove the box before sending.
