Healthcare CV template for nursing and medical agencies
A healthcare CV for an agency sets out what a trust or care provider checks first: registration and compliance. This Word template puts a registration card at the top left with the professional register, PIN or registration number, revalidation, DBS and indemnity exactly as stated, then clinical areas, mandatory training with completion dates, education and languages. Beside it: the profile, clinical skills, employment by trust, ward or unit and band, and clinical rotations or placements. Formatix fills it from the CV and your notes and never infers a registration status, a revalidation date or a training date.
Healthcare clients will not read past a CV that hides the registration. Agencies end up retyping it into their own format for every booking. Here registration, checks and training sit where the client looks first, and the career is laid out by trust and unit.
10 free credits, no card needed. One credit fills one document.
An editable file with neutral branding and a fictional example candidate, to fill in by hand. Formatix fills it automatically from a CV.

In Formatix the template is rebuilt in your brand, with your logo, colours and fonts taken from your website, and filled from a CV or your notes in about a minute. What you keep is the finished document: an editable Word, PowerPoint or Excel file.
Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. If you already have a document you send clients, upload that instead and Formatix turns it into a template. How Studio builds templates
Questions about this template
- Will it show the PIN or GMC number?
- Only if the CV or your notes give it. If your notes say the number should not be printed, the card says it is shared on request. Formatix never infers a registration status from a job title.
- What about mandatory training?
- Each course appears with its completion date as stated. A missing date stays missing; Formatix never estimates a date or an expiry.
- Does it work for doctors?
- Yes. The card takes the GMC number, training number and revalidation, and the rotations table lists training posts or placements.
- Is the candidate's contact information shown?
- No. Their phone, email and address are never printed; the client comes back to you.
- Can I download this template?
- Yes. A free, editable Word version with neutral branding and three fictional example candidates is on this page, to fill in by hand. To have it rebuilt in your brand and written automatically from CVs, use it in Formatix; what you keep is an editable Word file.
