Care worker profile
A care worker profile is the one page a care agency sends to care homes, home care and social care clients with a booking. Formatix fills it from the worker's CV and your notes, lists a shift or care setting only when the documents support it, lists only the training the worker has, and keeps every worker to one page.

What it is
Care clients decide on a booking from a few facts: availability, shifts, DBS, qualifications, training and whether the worker drives.
The Formatix template puts the name, role, base and availability at the top, then shifts, DBS, driving and qualifications answered in words, a short summary, care settings, experience and languages, mandatory training with dates and up to three recent roles. It fits exactly one page per worker.
Nothing beyond the CV and your notes is filled. A value that is not stated reads "Not stated". A closing line asks the client to confirm DBS and training records at booking.

- Is there a list of what is missing?
- Not a separate one. A value that is not stated reads "Not stated", so the gaps show on the page.
- Will it assume an enhanced DBS?
- No. DBS level, workforce and update service appear only as stated. An enhanced DBS or a barred list check is never assumed.
- Does it work with our ATS or CRM?
- JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works with Formatix by email: send the worker's CV and your notes to [email protected] and the filled document comes back.
