Healthcare and locum agencies
Locum CV compliance review
A locum CV compliance review checks a locum doctor's CV against what trusts require in the CV before the agency submits it. Formatix marks each requirement Present, Missing or Unclear from the CV and its attachments, quotes the evidence or says what is missing, and the template ticks Ready to submit only when every requirement is Present.

What it is
Trusts expect a locum doctor's CV to show certain things before the doctor is deemed suitable: full clinical and specialty experience with dates, every gap explained, GMC registration details, a current enhanced DBS, two recent references and hepatitis B evidence. Agencies check this before they submit.
The Formatix template is a generic review of twelve requirements in four groups (identity and availability, clinical experience, registration, pre-employment checks). Each has a Present, Missing or Unclear tick and an evidence column, followed by the outcome, a list of what to get from the doctor and a blank Reviewed by sign-off.
The AI marks each requirement; the template, not the AI, decides the outcome.
What Formatix fills in
| Field | Filled from |
|---|---|
| Doctor, specialty, agency, trust, grade, booking dates, duty covered, supervision | The CV for the doctor; the booking or your notes for the rest |
| Full name and availability for the dates needed | The CV, compared with the booking dates when given |
| Clinical experience, specialty experience, gaps explained | The CV |
| GMC registration type, number, currency and restrictions | The CV or an attached register print-out |
| Enhanced DBS, two references, hepatitis B evidence | The CV and attached documents |
| Evidence for each requirement | A short quote from the CV, the name of the attached document, or what is missing |
| Outcome: Ready to submit or Not yet | Worked out by the template: Ready only when every requirement is Present |
What stays blank
| Field | Why it stays blank |
|---|---|
| Reviewed by: reviewer, signature, date | Left blank for the person who reviews the CV; only the agency name is filled, from your notes. |
| A requirement the documents give nothing to judge | Left blank rather than guessed, when it is not something the CV should state. |
| Present, when the evidence is only usual for the grade | Never ticked: Present means the CV or an attachment clearly shows it. |
The Still needed list
When any requirement is Missing or Unclear, the review prints a "Still needed from the doctor" box with one line per requirement, most important first: for example an explanation of a gap, a second reference covering the last two years or a hepatitis B laboratory report. Get them from the doctor, then review the CV again before submitting.
Frequently asked questions
- When is a requirement Unclear rather than Missing?
- When the evidence is partial, out of date or contradictory: one reference only, a reference covering work more than two years ago, "Hep B immune" without a laboratory report, or availability without dates.
- Is this an official checklist?
- No. It is a generic Formatix template for suppliers submitting locum doctors to NHS trusts, and it says so in its footer.
- Does it work with our ATS or CRM?
- JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works with Formatix by email: send the doctor's CV and attachments to [email protected] and the filled document comes back.
