NHS Employment Check Standards checklist
The NHS Employment Check Standards, published by NHS Employers, set out the six checks NHS organisations expect before someone works for them, including staff supplied by an agency: identity, criminal record, work health assessment, professional registration and qualifications, right to work, and employment history and references. This Word template is a supplier's checklist for one agency worker against those six standards, in the order NHS Employers lists them (July 2026), in your brand: the booking, then each standard with its details (for example the DBS level, barred lists, certificate number and Update Service check, or the regulator and register check date), the supplier's confirmation and a Still needed list. Formatix fills it from the worker's CV and your compliance file, and a check is filled only where your file confirms it. It is a Formatix template, not an official NHS form, and it carries no NHS logo.
Trusts and their managed service providers ask agencies to show the six checks for every worker, and the evidence sits in different places: the DBS certificate, the OH clearance, the register check and the reference file. This checklist pulls those into one document per worker. A CV line such as "Enhanced DBS" or "NMC registered" can fill a detail box, but the check itself is left empty unless your record confirms it.

A CV line can fill a detail but never ticks a check. Not an official NHS form and carries no NHS logo.

Questions about this template
- Is this an official NHS form?
- No. It is a Formatix template laid out on the six NHS Employment Check Standards published by NHS Employers. Its footer says it is not an official NHS form, and it carries no NHS logo. If a trust gives you its own checklist, upload that and Formatix builds the template from it.
- Will it tick DBS because the CV mentions it?
- No. A CV line can fill the DBS details, but the check is marked only when your compliance record confirms it. A check that is not mentioned stays empty; it is never marked No by default.
- Does Formatix check the register or the DBS Update Service?
- No. It copies what your file says, character for character for registration and DBS numbers. The checks themselves stay with your compliance team.
- What about workers without a professional registration?
- For a healthcare support worker, the registration rows stay empty rather than being filled with a guess.
- Which health details go on it?
- Only the outcome of the work health assessment and the immunisation and screening status your OH record states. No diagnosis or other health detail, and no date of birth, document number or National Insurance number.
