    End-of-assignment report template

An end of assignment assessment report is the Buyer's assessment of a temporary worker when an assignment finishes, which the supplier keeps on the worker's file and shares with the worker. This Word template follows the structure of the RM6281 clinical and RM6277 non-clinical end of assignment reports in one design, in the agency's brand: report details (date, supplier, buyer and reference, position supplied, assignment dates); overall performance (Excellent, Good, Satisfactory or Unsatisfactory); whether the Buyer would engage the worker again; comments and training needs; the Buyer's reporting officer; the worker's statement; and the detailed assessment of fourteen criteria, each rated Above average, Average, Below average or Unacceptable. Formatix fills it from the Buyer's completed form, email or your notes of a call with the reporting officer. Every rating is the Buyer's own; a criterion the Buyer did not rate stays blank. It is a Formatix template, not an official form.

Feedback at the end of an assignment rarely arrives on the form. It comes as an email from the ward manager or a quick call, and someone has to write it up. This template turns that feedback into the full report without adding to it: a rating is ticked only when the Buyer gave it, comments keep the Buyer's words, and anything still missing is listed for the supplier to collect.

    Questions about this template

    Is this an official form?
No. It is a Formatix template for suppliers reporting to NHS and public sector buyers under the RM6281 or RM6277 frameworks, and its footer says it is not an official form. The structure follows those frameworks' end of assignment reports, published under the Open Government Licence v3.0, with no NHS, framework or government logo.
Will it fill in ratings the Buyer did not give?
No. Each of the fourteen criteria is rated only when the Buyer rated it. A rating is never inferred from the tone of a comment or from the overall rating, and ratings are never averaged. Criteria the Buyer did not rate stay blank and go on the Still needed list.
What if the overall performance is Unsatisfactory?
Then the full detailed assessment is needed, so every criterion the Buyer has not rated is listed in the Still needed box for the supplier to obtain.
    Who signs it?
The Buyer's reporting officer and the worker. Their names, positions and dates fill only when the documents give them; the signatures are always left blank.
