Interview feedback form template
An interview feedback form records what the interviewer thought of a candidate after an interview, so the recruiter can pass a decision back to the client's team and to the candidate. This Word template puts it on one page in the firm's brand: the candidate, role and client; who interviewed, the stage, date and format, and who took the feedback; the outcome as a choice of Proceed, Hold or Decline; the interviewer's overall impression; a rating against each of the role's criteria (Exceeds, Meets, Partly meets or Does not meet) with the interviewer's comment; what went well; the concerns raised; the agreed feedback for the candidate; and next steps. Formatix fills it from the recruiter's notes of the feedback call, and a criterion the interviewer did not judge is shown as "Not rated".
Most feedback after an interview arrives as a phone call or a two-line email, and the recruiter writes it up for the file, the client's HR team and the candidate. This template turns those notes into one clear, branded page every time. The ratings and opinions stay the interviewer's: nothing is scored by the template, and nothing is rated that the notes do not judge.
10 free credits, no card needed. One credit fills one document.
An editable file with neutral branding and a fictional example candidate, to fill in by hand. Formatix fills it automatically from a CV.

What you keep is the finished document: an editable Word, PowerPoint or Excel file.
Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. If you already have a document you send clients, upload that instead and Formatix turns it into a template. How Studio builds templates
Questions about this template
- What should an interview feedback form include?
- Who interviewed the candidate, for which role and at which stage; a clear outcome; the interviewer's overall impression; a view on each of the role's criteria with the reason for it; strengths and concerns; and the next step. This template also carries the agreed wording to pass back to the candidate, so what the candidate hears matches what the client said.
- Will it invent a rating?
- No. Each criterion is rated only when the interviewer's feedback judges it, on a plain scale of Exceeds, Meets, Partly meets or Does not meet. A criterion the interviewer did not cover is printed as "Not rated". The template never computes or averages a score, and the outcome is left undecided when the notes give no decision.
- How is it different from the interview scorecard?
- The interview scorecard is the interviewer's own scoring: competencies rated 1 to 5 with rating bars, an overall mean and a hire recommendation. The interview feedback form is the recruiter's record of the feedback: who gave it and when, a Proceed, Hold or Decline outcome, the interviewer's comments against each criterion and the wording agreed for the candidate.
- Can I send the feedback to the candidate?
- The form keeps the internal view and the candidate's version apart. The "Feedback for the candidate" panel holds only wording the notes say can be shared, or the interviewer's feedback put neutrally, so you can copy it into an email or read it out on the call. The rest of the form stays with the hiring team.
- Can I download this template?
- Yes. A free, editable Word version with neutral branding and three fictional example candidates is on this page, to fill in by hand. To have it rebuilt in your brand and filled automatically from your feedback notes, use it in Formatix; what you keep is an editable Word file.
