Site operative compliance checklist
A site operative compliance checklist is the record a labour agency completes before an operative starts on site. Formatix fills it from the operative's documents and your notes, ticks a check only when your record says it was done, never assumes a test result, and lists every expired card and missing check before the start.
No credit card. Your first ten CVs are free, and the trial credits are valid for 14 days.
What it is
Main contractors and their supply chain teams expect agencies to show the checks behind each operative: identity and right to work, a CSCS Smart Check of the card, plant cards, site safety tickets, a drug and alcohol test where the client requires one, and the site induction.
The Formatix template covers the booking; identity and right to work (manual, online with a share code, or a digital identity check); the CSCS card with the Smart Check date and result; plant cards with categories and whether they were verified; tickets such as SMSTS, SSSTS, first aid, asbestos awareness, PASMA or IPAF; fitness for work; site induction and PPE; references; and a Still needed box.
A photo of a card is not a Smart Check, and a CV line is not a ticket. The checklist fills only from what the documents show.
What Formatix fills in
| Field | Filled from |
|---|---|
| Client, site, location, start date, booking reference, shift pattern, reporting to | The booking notes |
| Client requires a drug and alcohol test | The booking notes, as stated |
| Identity document, right to work check type, date, who checked, follow-up date | Your right to work record |
| CSCS card type, number, expiry, Smart Check date and result | The card and your Smart Check record |
| Plant cards and tickets with numbers, categories, dates and expiry | The card scans and certificates |
| D&A test date, provider, result; health surveillance | The test record, as recorded |
| Site induction, PPE issued, client requirements, references | Your notes and the reference replies |
What stays blank
| Field | Why it stays blank |
|---|---|
| Smart Check, when only a photo of the card was given | Left empty. A photo is not a Smart Check. |
| A test result not recorded | Never assumed to be negative. |
| A Yes or No not stated | Left empty, never No by default. |
| Date of birth, address, phone, email, National Insurance number, medical details | Never written, beyond the health surveillance line as stated. |
The Still needed list
The checklist ends with a "Still needed before the operative starts" box: every check the documents do not give (for example a drug and alcohol test the client requires, the site induction date or a second reference) and every expired card or ticket, named with its expiry date.
Frequently asked questions
- Is this an official contractor form?
- No. It is a Formatix template, and its footer says it is not an official form. If your client issues its own pre-start checklist, upload that instead.
- Does Formatix run the Smart Check or the right to work check?
- No. It records the checks you made, from your records.
- Does it work with our ATS or CRM?
- JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works with Formatix by email: send the CV and your notes to [email protected] and the filled document comes back.
