Rail operative profile template (PTS / Sentinel)
A rail operative profile is what a rail labour agency sends a contractor with a track operative, built around what the contractor checks before the operative goes on or near the line. This Word template sets it out in your brand: name and rail role; the Sentinel number and Primary Sponsor; the rail medical and the last drug and alcohol screening as stated; availability, base and shifts; the competences on the operative's Sentinel profile, such as PTS AC, PTS DCCR, COSS, Lookout or Handsignaller, each with its expiry; rail experience, most recent first; and the CSCS card, other tickets and driving licence. Formatix fills it from the CV, the Sentinel profile print and your notes. A competence is never added because the role usually needs it, expiries are copied rather than worked out, and a lapsed competence is marked. It is a Formatix template, not an official form.
Rail contractors read a track operative's profile for the Sentinel number, the sponsor and the competences in date. This template puts those first and takes them from the Sentinel print rather than the CV. The Primary Sponsor is written as stated, never assumed to be your agency, and shifts are shown only as the operative or your notes give them.

What you keep is the finished document: an editable Word, PowerPoint or Excel file.
Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. Questions about this template
- Where do the competences come from?
- Only from the Sentinel profile, certificates or your notes. A competence is never added because the role usually needs it, and an expiry is never worked out from a course date.
- Does it work out the medical expiry?
- No. The rail medical is copied as stated, for example "Sentinel medical, valid to 14/05/2028". The template never calculates it from the operative's age or the date of the medical.
- How is a lapsed competence shown?
- As Expired, only when the documents say it has lapsed or its stated expiry date is earlier than the date of your notes.
- Does Formatix check Sentinel?
- No. It copies what the Sentinel print and your notes say. The contractor still checks the operative's card and competences on Sentinel.
- What personal details stay off?
- Date of birth, home address, phone, email, National Insurance number and medical details beyond the medical line are never printed.
