Key Information Document template for agency workers
A Key Information Document (KID) is the document an employment business must give an agency worker before they agree terms, under regulation 13A of the Conduct of Employment Agencies and Employment Businesses Regulations 2003. It sets out who employs and pays the worker, the type of contract, the rate of pay, deductions, fees, holiday and benefits, and an example of pay for one pay period. This Word template follows the rows of the GOV.UK example KIDs (updated 7 April 2026) in your brand, with a version for each pay route: PAYE, umbrella company (with the umbrella company's gross rate and deductions) and the worker's own limited company (with the personal service company opt-out note). Formatix fills it from your terms for the worker. Every amount is copied exactly as your terms give it and nothing is worked out. It is a Formatix template, not an official form, and it does not give legal advice.
Most compliance teams keep a KID as a Word file per pay route and retype the worker's name, rate and example pay for every new starter. This template takes those figures from the terms you already have for the worker. If the terms do not give a row, such as holiday pay or the umbrella margin, the row stays empty and goes on a Still needed list at the end, which says to remove it before the KID is issued.

What's in the template

In Formatix the template is rebuilt in your brand, with your logo, colours and fonts taken from your website, and filled from a CV or your notes in about a minute. What you keep is the finished document: an editable Word, PowerPoint or Excel file.
Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. If you already have a document you send clients, upload that instead and Formatix turns it into a template. How Studio builds templates
Questions about this template
- Is this the official GOV.UK Key Information Document?
- No. GOV.UK publishes guidance and example KIDs, not a form you must use. This template follows the rows of those examples (updated 7 April 2026, Open Government Licence v3.0) and its footer says it is not an official form and does not give legal advice. Check the current guidance and take advice on your own terms where you need it.
- Does Formatix work out the example net pay?
- No. Gross pay, deductions, fees and net take home pay are copied exactly as your terms or example payslip give them. Formatix never calculates tax, National Insurance or an umbrella margin, and never uses a typical rate or the National Living Wage. A missing figure stays blank and goes on the Still needed list.
- Which pay routes does it cover?
- PAYE through the employment business, an umbrella company, and the worker's own limited company. The umbrella rows (the umbrella company, any business connection, the gross rate transferred and its deductions) fill only on the umbrella route, and the opt-out note only on the limited company route, as your notes state.
- Does it keep to two pages?
- Yes. Regulation 13A allows two sides of A4 per worker, and the template was tested in Microsoft Word to stay within that.
- What personal details stay off the KID?
- Date of birth, home address, National Insurance number and bank details are never put on it.
