Temporary and contract staffing
Key Information Document for agency workers
A Key Information Document (KID) is what an employment business must give an agency worker before they agree terms: who employs and pays them, the contract type, the rate of pay, deductions, fees, holiday, benefits and an example of pay. Formatix fills it from your terms for the worker, copies every figure exactly, and lists any row your terms do not give before the KID is issued.
No credit card. Your first ten CVs are free, and the trial credits are valid for 14 days.
What it is
Regulation 13A of the Conduct of Employment Agencies and Employment Businesses Regulations 2003 requires an employment business to give a work-seeker a Key Information Document before agreeing terms with them. It must be headed "Key Information Document", and it can run to two sides of A4.
The Formatix template follows the rows of the GOV.UK example KIDs (updated 7 April 2026): the worker, version and date issued; the further information line and the Fair Work Agency statement; general information (employment business, employer, contract type, who pays, how often, rate of pay, deductions, fees, holiday, benefits); the umbrella company pay information on the umbrella route, or the personal service company note on the limited company route; and the example pay for one pay period.
Every figure is copied from your terms. Nothing is calculated, estimated or taken from a typical rate. Anything missing goes on a Still needed box that says to delete it before the KID is issued.
What Formatix fills in
| Field | Filled from |
|---|---|
| Worker's name, version (draft, final or revised), date issued | Your notes, as stated |
| Where further information can be found | Your notes, such as the terms of engagement or worker handbook |
| Employment business, employer, who pays, how often | Your terms for the worker |
| Type of contract | Your terms: contract of service, apprenticeship or contract for services, only as stated |
| Rate of pay, deductions required by law, other deductions, fees | Your terms, copied exactly with £ and pence |
| Holiday entitlement and pay, additional benefits | Your terms, as stated |
| Umbrella company, business connection, gross rate transferred, umbrella deductions | Your terms, on the umbrella route only |
| Example pay: period, gross, deductions, fees, net take home pay | Your example or payslip, copied exactly |
What stays blank
| Field | Why it stays blank |
|---|---|
| Any row your terms do not give | It stays empty and goes on the Still needed list. "None" is written only when your terms say there is none. |
| Net pay, tax, National Insurance or umbrella margin you have not stated | Formatix never calculates pay and never uses the National Living Wage, a tax table or a usual margin. |
| Date of birth, home address, National Insurance number, bank details | Never put on the KID. |
The Still needed list
The KID ends with a dashed "Still needed before this KID is issued" box for the agency only. It lists, most important first, every row your terms do not give: for example the rate of pay, who pays the worker, holiday entitlement and pay, the example pay or the umbrella company's deductions. Fill them in, then delete the box: regulation 13A allows only the regulation's content in the KID itself.
Frequently asked questions
- Is this the official GOV.UK KID?
- No. GOV.UK publishes guidance and example KIDs rather than a form you must use. This template follows the rows of those examples and its footer says it is not an official form and does not give legal advice.
- Does Formatix work out the example net pay?
- No. Every figure in the example pay is copied from your terms or payslip. If a figure is missing, the row stays empty and goes on the Still needed list.
- Can we use our own KID layout?
- Yes. Upload the KID your agency already issues and Formatix builds the template from it, keeping its layout.
- Does it work with our ATS or CRM?
- JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works with Formatix by email: send the CV and your notes to [email protected] and the filled document comes back.
