Temporary and contract staffing
Agency worker suitability record
The agency worker suitability record shows the checks an employment agency or employment business makes before it introduces or supplies a work-seeker, in the order of regulations 18 to 22 of the Conduct Regulations 2003. Formatix fills it from the CV, the hirer's booking and your compliance file, answers Yes or No only where your file says so, and lists everything still to do before the worker is put forward.
No credit card. Your first ten CVs are free, and the trial credits are valid for 14 days.
What it is
The Conduct Regulations set out what an agency must find out from the hirer, what it must confirm about the work-seeker, the steps it takes to protect both, what it passes on, and the extra steps for work with vulnerable people.
The Formatix template records them for one worker and booking: information from the hirer (regulation 18), identity, required qualifications and authorisations and willingness (regulation 19), protection of the work-seeker and the hirer (regulation 20), information given (regulation 21), vulnerable persons and required qualifications with two references from non-relatives (regulation 22), who completed it, and a Still needed box.
A CV line is treated as the worker's claim, not as a confirmed requirement. Dates are copied as recorded.
What Formatix fills in
| Field | Filled from |
|---|---|
| Hirer, nature of business, position, start date, duration, duties, location, hours, health and safety risks, expenses | The hirer's booking or your notes of it, never the CV |
| Acting as employment agency or employment business | Your notes, as stated |
| Identity confirmed, how and when | Your compliance file |
| Each requirement, required by (hirer, law, professional body), evidence seen | The booking, the CV and your file |
| Requirement confirmed | Yes only when your notes confirm the worker meets it |
| Willingness, protection steps, information given and when | Your notes, as recorded |
| References for vulnerable-person roles, consent to disclose | References actually received, as recorded |
| Completed by, position and date | Your notes, never today's date by default |
What stays blank
| Field | Why it stays blank |
|---|---|
| A Yes or No your file does not record | It stays empty. It is never No because it is not mentioned, or Yes because the check is usual. |
| Health and safety risks the hirer has not given | Left blank; "None known" only when the hirer said so. |
| The date for written confirmation | Copied as recorded; the third business day is never calculated. |
| Document numbers, date of birth, National Insurance number, health details | Never written on the record. |
The Still needed list
The record ends with a "Still needed before introduction or supply" box. It lists, most important first, what the file does not yet show: for example confirming identity, a second reference from a non-relative with consent to disclosure, the hirer's health and safety risks, a copy of a licence to offer the hirer, or the worker's confirmation that they are willing to do the work.
