    Agency social worker reference

The agency social worker reference is the standard reference a referee at a local authority gives for an agency child and family social worker. Formatix lays out the referee's own answers from the returned form, their email or your notes of a reference call, never writes an answer or rating the referee did not give, and lists what you still need to chase.

    What it is

Agencies supplying child and family social workers to local authorities in England obtain a standard reference for each placement. The Department for Education publishes a standard reference template: a basic reference (the social worker, the assignment and the referee) and a detailed reference with A to D ratings against twelve competency statements, followed by fitness to practise, re-employment and development needs.

The reference is completed by the referee at the local authority, not by the agency. The Formatix template follows the standard structure in your brand, without government branding, so the referee's answers are recorded consistently, and adds a Still needed box for what the referee has not yet answered.

Formatix only sets out what the referee wrote or said. The social worker's CV is used for their own details, never for a rating or an answer.

    What stays blank

    FieldWhy it stays blank
Referee's signatureAlways left blank for the referee.
A rating the referee gave in words but not as a letterFormatix never turns comments into a rating; the rating stays blank and goes on the Still needed list.
Any question the referee did not answerLeft blank. A written answer the referee did not give would be a serious error.

    The Still needed list

The reference includes a "Still needed from the referee" box for the agency, listing, most important first, every part of the reference the documents do not give: for example ratings not given, an unanswered fitness to practise question, the signed and dated declaration or the referee's email. Chase them with the referee, then remove the box before sending.

    Frequently asked questions

Is this the official DfE template?
No. It is a Formatix template for suppliers submitting agency social workers to local authorities, and it says so in its footer. The structure follows the Department for Education's standard reference template for agency social workers, published under the Open Government Licence v3.0.
Can Formatix write the reference for us?
No. The reference is the referee's statement. Formatix sets out what the referee wrote or said and leaves everything else blank.
Does it work with our ATS or CRM?
JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works with Formatix by email: send the referee's answers to [email protected] and the filled document comes back.
