    Agency social worker reference template

The agency social worker reference is the standard reference for an agency child and family social worker, completed by a referee at a local authority where the social worker has worked. This Word template follows the structure of the Department for Education's standard reference template for agency social workers, in the agency's own brand: the social worker and their Social Work England number; the assignment (job title, local authority, dates, area of practice and court work); the referee's details; the A to D ratings against twelve competency statements in four groups; fitness to practise concerns; whether the referee would re-employ the social worker; development needs; additional information; the referee's declaration with a blank signature line; and a Still needed list. Formatix lays out the referee's own answers, from the returned reference or your notes of a reference call. It never writes an answer or a rating the referee did not give. It is a Formatix template, not an official form.

References for agency social workers often come back as an email, a partly completed form or a phone call. This template sets the referee's answers out in the standard structure so the agency has a complete, consistent record, and shows exactly what the referee has not yet answered. The ratings and answers are always the referee's: the CV is used only for the social worker's own details.

    Questions about this template

    Who completes the reference?
The referee, at the local authority. Formatix does not write the reference: it sets out the referee's own answers from the returned form, their email or your notes of a reference call, and leaves blank anything the referee did not answer.
Will it turn the referee's comments into a rating?
No. A rating of A, B, C or D is ticked only when the referee gave that letter. If the referee gave words but no letter, the rating stays blank and goes on the Still needed list.
What does the CV fill in?
Only the social worker's own details, such as their name and Social Work England number, and the assignment's job title, local authority and dates when the referee does not give them. Never a rating or an answer.
Is this the official DfE template?
No. It is a Formatix template for suppliers submitting agency social workers to local authorities, and its footer says it is not an official form. The structure follows the Department for Education's standard reference template, published under the Open Government Licence v3.0, with no government branding.
