    Agency social worker compliance checklist

Before a locum social worker starts with a council in England, the agency checks their Social Work England registration, DBS, identity, right to work and references, and for child and family work the DfE agency rules. This Word template sets those checks out for one social worker in your brand: the assignment (council, team, practice area, dates, hours, notice and the agreed rates); Social Work England registration, the register check, any conditions or fitness to practise order, AMHP and BIA annotations, the qualification and ASYE; for child and family work, the DfE agency rules (three years' post-qualifying experience in local authority employment, whether they left a substantive role nearby in the last three months, and two detailed references on the standard template); the enhanced DBS and Update Service; identity and right to work; references; other checks; the agency declaration; and a Still needed list. Formatix fills it from the CV and your compliance file. It is a Formatix template, not an official form.

Councils ask the same compliance questions about every locum, and the DfE agency rules added more for child and family work. This checklist keeps them on one page set per social worker. It never works out post-qualifying experience from the CV's dates: it puts what the CV says in the details and leaves the check for your record, with anything unclear on the Still needed list.

    Questions about this template

Is this an official council or DfE form?
No. It is a Formatix template that sets out the Social Work England registration details and the DfE agency rules (statutory guidance in effect from 31 October 2024). Its footer says it is not an official form. If a council gives you its own checklist, upload that and Formatix builds the template from it.
Will it write an HCPC number?
No. Social workers in England have been registered with Social Work England since December 2019. The checklist uses the SW number and never writes an HCPC number as the registration.
Does it work out three years' post-qualifying experience?
No. That box is filled only when your check records it. What the CV says goes in the details, and when it is unclear or under three years, it goes on the Still needed list.
Does it fill in the rates?
Only as agreed in your notes, with the basis (limited company, umbrella or PAYE). A rate is never estimated.
