School confirmation of agency staff checks (KCSIE)
Keeping Children Safe in Education asks schools and colleges to obtain written notification from an agency that it has carried out the same checks the school would carry out on its own staff, including who obtained the enhanced DBS certificate and whether it disclosed anything. This Word template is that notification, set out on KCSIE 2026 (paragraphs 364 to 367), in your agency's brand: the supply worker and the booking; each check with the date completed and its details (identity, enhanced DBS with children's barred list information, prohibition from teaching, qualifications, right to work, overseas checks, fitness, references, and childcare disqualification and section 128 where they apply); the enhanced DBS certificate and who obtained it; the worker's photo for the school's identity check on arrival; the agency's confirmation; and a box for the school to complete for its single central record. Formatix fills it from your vetting record. It is a Formatix template, not an official form.
A school relies on this letter instead of doing the checks itself, so the dates and details have to match your vetting record exactly. This template copies them from that record and leaves a check empty if the record does not confirm it, even when the CV mentions it. Anything still to obtain is listed at the end for the agency, and the signature is always left for a person to sign.

Questions about this template
- Is this an official DfE form?
- No. KCSIE asks for written notification but does not prescribe a form. This template follows paragraphs 364 to 367 of KCSIE 2026 and its footer says it is not an official form. It carries no DfE branding.
- Will it confirm a check because the CV mentions it?
- No. A CV line such as "Enhanced DBS" or "QTS 2016" can go in the details, but the check is confirmed only when your vetting record says the agency carried it out, or that it does not apply to this role.
- Why is there a photo on it?
- KCSIE asks schools to check that the person who arrives is the person the agency checked. The worker's photo is printed in the On arrival box for that check, when the CV or your file has one.
- Who signs it?
- The agency. The signature is always left blank, and the date is filled only when your notes give it, never today's date by default.
- What does the school fill in?
- The single central record box: the date the confirmation was received, the DBS details provided, the identity check on arrival and who recorded it. Formatix leaves it blank for the school.
