Supply teacher profile template
A supply teacher profile is the one page a supply agency sends a school that is booking cover. This Word template keeps every teacher to one page in the agency's brand: the name and headline, with the photo when the CV has one; subjects, key stages and availability (day-to-day, short-term or long-term, the days and the start date); teaching status (QTS, QTLS or ECT, with the year and route as stated), the DBS and whether it is on the update service, and safeguarding training with its date; a short profile; SEN and SEND experience; strengths in the classroom; the schools taught at with type, phase, dates and role; and a Still needed list. Formatix fills it from the teacher's CV and your notes, and every status, check and date appears only as stated.
A school booking cover wants to know four things fast: can they teach this subject and key stage, are they qualified, are the checks in order, and when can they start. This page answers them at the top, from the teacher's documents only. A PGCE is never treated as QTS, and the DBS certificate number is never printed.

Download or automate? The free download is a plain file you fill in by hand. In Formatix the template is rebuilt in your brand, with your logo, colours and fonts taken from your website, and filled from a CV or your notes in about a minute. What you keep is the finished document: an editable Word, PowerPoint or Excel file.
Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. If you already have a document you send clients, upload that instead and Formatix turns it into a template. How Studio builds templates
Questions about this template
- Will it show QTS for a teacher with a PGCE?
- No. QTS, QTLS or ECT appear only as stated. A PGCE alone is not QTS, and an overseas teaching qualification is shown as it is, not as QTS, unless the documents say QTS was awarded.
- How are key stages worked out?
- Only from key stages the CV states or the year groups it names, such as Years 7 to 9 for KS3. A key stage is never inferred from a school's phase alone.
- What does the DBS line show?
- The DBS type and issue date as stated, and whether it is on the update service only when the documents say so. The certificate number is never printed, and an old DBS is never treated as current.
- What goes on the Still needed list?
- Up to four things the school will expect that the documents do not give, such as the teaching status and year awarded, the DBS update-service status or the date of the latest safeguarding training. Unstated answers on the page read "Not stated".
