    Supply teacher compliance checklist template

A supply teacher compliance checklist is the form a supply agency sends a school or academy trust when it confirms a booking, showing who the worker is, what the booking costs and which employment checks the agency has completed. This Word template follows the structure of the Temporary Worker Compliance Checklist in Annex C of the RM6376 Supply Teachers and Education Recruitment framework, in the agency's own brand: the booking (buyer, school, reference, job title, subject, placement dates, working pattern and days booked); the worker's name, a recent photograph and whether they have the skills for the role; the worker charge rate before and after AWR, the agency fee, the total daily charge and the payment type; the employment checks (identity, right to work, QTS or QTLS, employment history and references, DBS, barred list, prohibition) with their details; the buyer's extra requirements; the supplier declaration and sign-off; and a Still needed list. Formatix fills it from the worker's CV and your compliance notes. It is a Formatix template, not an official form.

Every booking under the framework can need the same checklist, and the facts for it sit in three places: the worker's CV, the agency's vetting record and the booking from the school. This template brings them together on one form. A check is ticked only when your records say the agency completed it, rates are copied exactly as agreed, and anything missing is listed for you to obtain before the booking is confirmed.

    Questions about this template

Is this the official RM6376 form?
No. It is a Formatix template for suppliers submitting to schools and academy trusts under the RM6376 framework, and its footer says it is not an official form. The structure follows Annex C of the framework specification, published under the Open Government Licence v3.0. It carries no crest, framework logo or DfE branding. Check the version your buyer asks for before you submit.
Will it tick a check because the CV mentions it?
No. A CV is not a compliance record. A line such as "Enhanced DBS on the Update Service" can fill the DBS details, but the check itself is ticked only when your notes or compliance record say the agency completed it. A check that is not mentioned stays unticked; it is never marked No by default.
Does it work out the charge rate after AWR?
No. The worker charge rate before and after AWR, the agency fee and the total daily charge are copied exactly as stated. Formatix never adds a fee to a rate, never works out a post-AWR rate and never takes a rate from the role or the region. Days booked are never counted from the dates.
    Who signs it?
The supplier. The signature box is always left blank for a signature by hand or e-signature. The supplier name, the signatory's name and position, and the date fill only when your notes give them; the date is never set to today by default.
What personal details stay off the checklist?
Date of birth, home address, National Insurance number, passport number, share code, nationality and health information are never put on it.
