    Agency driver file template

An agency driver file brings together what a driver agency holds before a driver starts with a client. This Word template covers the assignment, identity and right to work, the DVLA licence check, Driver CPC card, digital tachograph driver card, other tickets, time worked for other employers, assessment and references. Formatix fills it from the CV, the check result, card scans and your notes. Categories come from the licence check, never from the CV alone, and only the last 8 characters of the licence number are held. It is a Formatix template, not an official form.

The details sit in a check result, card photos and a phone note. This file puts them in one place. A photo of a licence is not shown as a DVLA check, and nothing is marked International unless the document says so.

Download or automate? The free download is a plain file you fill in by hand. In Formatix the template is rebuilt in your brand, with your logo, colours and fonts taken from your website, and filled from a CV or your notes in about a minute. What you keep is the finished document: an editable Word, PowerPoint or Excel file.

Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. If you already have a document you send clients, upload that instead and Formatix turns it into a template. How Studio builds templates

    Questions about this template

Does Formatix run the licence check?
No. You run it with the driver's check code on GOV.UK, and Formatix records the date and result you give it. A check code must be used within 21 days and only works once.
    Is that a share code?
For driving licences the term is a check code. A share code is the right to work service.
    Why ask about other employers?
Under the Road Transport (Working Time) Regulations 2005 an employer of a mobile worker asks for time worked for other employers and counts it. DVSA guidance says agencies keep working time records where the driver is paid by or through them.
Does it work out expiry dates?
No. Driver CPC and tacho card expiries are copied from the cards.
