Security screening file template (for firms working to BS 7858)
SIA guidance tells approved contractors that outsourced screening should end with a file accounting for every stage, not simply a certificate. This Word template is that file: screening period and dates, consent, identity and address, right to work, SIA licence and register check, the career and activity history with who verified each period, how and when, any unverified periods, criminal record and financial checks, and the outcome your firm recorded. Formatix fills it from your records. It does not reproduce BS 7858, a paid British Standard, and it does not decide whether anyone passes. Work from your own copy of the standard. It is a Formatix template, not an official form.
Screening runs over weeks, with replies arriving in different forms. This file keeps every stage in one place. A CV entry is never shown as verified: a period only counts when your notes say who confirmed it.
10 free credits, no card needed. One credit fills one document.
An editable file with neutral branding and a fictional example candidate, to fill in by hand. Formatix fills it automatically from a CV.

Questions about this template
- Is this the BS 7858 form?
- No. BS 7858 is a paid British Standard and this template does not reproduce it. It records what you did. Buy the standard from BSI, or through the SIA's discount for approved contractors.
- Is BS 7858 a legal requirement?
- Not for every security firm. The SIA says firms in its voluntary Approved Contractor Scheme are required to work to BS 7858 or an appropriate sector equivalent. An SIA licence is a separate requirement for the person.
- What screening period does it use?
- The period your firm sets, typed in. NSI's summary of the 2019 edition gives five years as the minimum and ten where a longer period is required. Check your copy of the standard.
- Does Formatix run the checks?
- No. It records the checks you or your provider made. Criminal record and financial details are never repeated, only that the check was done and when.
