Safety critical work competency record template (ROGS)
Regulation 24 of the Railways and Other Guided Transport Systems (Safety) Regulations 2006 says a controller of safety critical work must make sure a person has been assessed as competent and fit by an assessor, and that there is an accurate, up-to-date written record referencing the criteria used. This Word template is that record: tasks, criteria, assessor, dates, fitness outcome, monitoring arrangements, reviews and who has seen it. Formatix fills it from your assessment records. A course certificate is never shown as an assessment. It is a Formatix template, not an official form.
The Rail Operative Profile tells a contractor who a person is. This record sits behind it and shows the assessment, with the criteria the regulation asks to be referenced. A missing criterion goes on the Still needed list.

Questions about this template
- Is every rail worker covered?
- No. Regulation 24 is about safety critical work as ROGS defines it. The template only treats a role as safety critical when your documents do.
- Does a PTS card count as an assessment?
- Not on this record. A course or card is copied as a course or card. The assessor field fills only where your documents name one.
- Does it hold medical details?
- No. Only the fitness outcome. Medical details stay with occupational health.
- Does it replace Sentinel?
- No. Sentinel stays your source for competences. The record adds the assessment detail, monitoring and reviews.
