Full employment history and gaps record template
Schedule 3 to the 2014 Regulations asks for a full employment history together with a satisfactory written explanation of any gaps. This Word template lays the history out with the most recent period first, shows each gap the stated dates leave, and sets the worker's own explanation beside it with where and when it was given. Formatix fills it from the CV, the application form and your notes. It never writes an explanation for the worker: a gap with none stays blank and goes on the Still needed list. It is a Formatix template, not an official form.
The gap is the part of a care file most likely to be missing. CVs round dates and skip months, and a gap is easy to read past. This record sets the periods out in order, so the gaps show, and keeps the worker's explanation in their own words.

Download or automate? The free download is a plain file you fill in by hand. In Formatix the template is rebuilt in your brand, with your logo, colours and fonts taken from your website, and filled from a CV or your notes in about a minute. What you keep is the finished document: an editable Word, PowerPoint or Excel file.
Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. If you already have a document you send clients, upload that instead and Formatix turns it into a template. How Studio builds templates
Questions about this template
- Does Formatix explain the gaps?
- Never. It copies what the worker wrote. If there is no written explanation the box stays blank and the gap goes on the Still needed list.
- How short a gap counts?
- Schedule 3 sets no minimum, so the template lists every gap the stated dates show and leaves the judgement to you.
- What if the CV only gives years?
- The dates are copied as written and never completed. If that means a gap cannot be told, it goes on the Still needed list.
- Does it work with our ATS or CRM?
- JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works by email: send the documents and your notes to [email protected] and the filled document comes back.
