Agency care worker confirmation template
CQC's guidance on Regulation 19 counts agency staff as persons employed, so a care home or home care provider needs to know what is held on each agency worker. This Word template is the document a care agency sends before the first shift: the booking, each check on the agency's file with its date and who made it, training with renewal dates, and anything still outstanding. Formatix fills it from your compliance records. It is the agency's own document, not a form set by CQC, and the provider keeps its own responsibilities.
Care providers ask for this in different ways, and agencies answer with a mix of emails and PDFs. One clear page per worker gets the booking moving and shows what is still to come.
10 free credits, no card needed. One credit fills one document.
An editable file with neutral branding and a fictional example candidate, to fill in by hand. Formatix fills it automatically from a CV.

In Formatix the template is rebuilt in your brand, with your logo, colours and fonts taken from your website, and filled from a CV or your notes in about a minute. What you keep is the finished document: an editable Word, PowerPoint or Excel file.
Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. If you already have a document you send clients, upload that instead and Formatix turns it into a template. How Studio builds templates
Questions about this template
- Is this form required by CQC?
- No. There is no CQC form for this. It is the agency's own summary of its file.
- Does it say a worker is cleared?
- No. It lists what the file holds, with dates. It does not say anyone is cleared unless that exact outcome is on your record.
- Does it repeat what a DBS certificate shows?
- No. It gives the level and date. What a certificate discloses is never printed.
- Does it work with our ATS or CRM?
- JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works by email: send the documents and your notes to [email protected] and the filled document comes back.
