Care staff compliance matrix (Excel)
A care staff compliance matrix shows, one row per worker, where each person's checks and training stand. This Excel template covers engagement, DBS level and date, right to work, history and references, and training renewals. A date turns red once it has passed and amber within 90 days of the as-at date, and only a full date is ever highlighted. Formatix fills each row from your records. Training intervals stay your own: a renewal date is only ever copied, never worked out. It is a Formatix template, not an official form.
One row per worker, built from the records you already hold. The red and amber are live, so the sheet keeps working as dates pass.
10 free credits, no card needed. One credit fills one document.
An editable file with neutral branding and a fictional example candidate, to fill in by hand. Formatix fills it automatically from a CV.

Download or automate? The free download is a plain file you fill in by hand. In Formatix the template is rebuilt in your brand, with your logo, colours and fonts taken from your website, and filled from a CV or your notes in about a minute. What you keep is the finished document: an editable Word, PowerPoint or Excel file.
Ask for changes in plain words, such as "move the photo to the right" or "add a Languages section", and the template is updated with the rest of its formatting kept. If you already have a document you send clients, upload that instead and Formatix turns it into a template. How Studio builds templates
Questions about this template
- Does it work out renewal dates?
- No. Training intervals are your own policy, so a renewal date is copied from your records, never calculated from a course date.
- What happens with a partial date?
- It is shown as written and not highlighted or counted. Only a full date turns red or amber.
- Does it work with our ATS or CRM?
- JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works by email: send the documents and your notes to [email protected] and the filled document comes back.
