Supply teacher compliance checklist
A supply teacher compliance checklist is the form a supply agency sends a school or academy trust when it confirms a booking, setting out the booking, the charges and the employment checks the agency has completed. Formatix fills it from the worker's CV, the booking and your compliance record, ticks a check only when your record says it was done, and lists what you still need before the booking is confirmed.

What it is
The RM6376 Supply Teachers and Education Recruitment framework includes a Temporary Worker Compliance Checklist (Annex C of the framework specification) that a supplier gives the buyer when it confirms a booking, where the buyer requires it. It covers the booking, the worker, the charges and the pre-employment checks, and the supplier signs it.
The Formatix template follows that structure in your own brand: the booking (buyer and location, school, buyer's reference, job title, subject specialism, placement dates, working pattern, days booked), the worker with a recent photograph and whether they have the skills for the role, the charge rate and payment, the employment checks with their details, the buyer's additional requirements, the supplier declaration and sign-off, and a Still needed box.
Ticks, rates, dates and numbers on this checklist come only from the worker's documents, the booking and your records. Anything missing is left blank and added to the Still needed list.
What Formatix fills in
| Field | Filled from |
|---|---|
| Buyer, school, buyer's reference, job title, subject, placement dates, working pattern, days booked | The booking in your notes, as stated |
| Worker's full name | The CV |
| Skills and knowledge for the role (Yes or No) and the supporting evidence | Yes or No only when your notes state it; the evidence from the CV and notes |
| Recent photograph | The CV's photo, when it has one |
| Worker charge rate pre and post AWR, agency fee, total daily charge, payment type | Your notes, copied exactly |
| Identity, right to work, QTS / QTLS, references, DBS, barred list and prohibition checks | Ticked only when your notes or compliance record confirm the agency completed the check |
| Check details: DBS disclosure type, certificate number and issue date, TRN or QTS details, comments | Your compliance record; a CV line can fill the details, but never the tick |
| Extra mandatory training and specific skills required by the buyer | Your notes, as stated |
| Supplier name, signatory's name and position, sign-off date | Your notes, only when given |
What stays blank
| Field | Why it stays blank |
|---|---|
| Signature | Always left for the supplier to sign by hand or e-signature. |
| A check your record does not confirm | It stays unticked. It is never marked No because it is not mentioned, or Yes because it is usual. |
| A rate, fee or total that is not stated | Formatix never adds a fee to a rate, works out a post-AWR rate or takes a rate from the role or region. |
| Days booked, when the booking does not state them | They are never counted from the placement dates. |
| Recent photograph, when the CV has none | The photo box reads "Attach a recent photograph" and the photograph goes on the Still needed list. |
| Date of birth, home address, National Insurance number, passport number, share code, nationality, health | Never put on this checklist. |
The Still needed list
The checklist ends with a "Still needed before confirming the booking" box for the agency. It lists, most important first, every item the checklist asks for that the CV and your records do not give: for example the DBS certificate number and date of issue, confirmation of the barred list or prohibition check, the right to work check, the charge rate after AWR, a recent photograph or the payment type. Obtain them, then remove the box before sending.
Frequently asked questions
- Is this the official RM6376 checklist?
- No. It is a Formatix template for suppliers submitting to schools and academy trusts under the RM6376 framework, and it says so in its footer. It follows the structure of Annex C, published under the Open Government Licence v3.0, and carries no crest, framework logo or DfE branding. Check the version your buyer asks for before you submit.
- Will it tick DBS because the CV mentions it?
- No. A CV line such as "Enhanced DBS on the Update Service" can fill the DBS details, but the check is ticked only when your notes or compliance record say the agency completed it.
- Does Formatix carry out the checks?
- No. It copies what your records say. The checks, and the decision to confirm the booking, stay with your compliance team.
- Does it work with our ATS or CRM?
- JobAdder, Salesforce, HubSpot and Ezekia connect to Formatix natively on every plan, including the free trial. Any other ATS or CRM works with Formatix by email: send the worker's CV and your notes to [email protected] and the filled document comes back.
